CASE MANAGEMENT AND LIFE CARE PLANNING
- KRISTI SCHAEFER: Owner of NeuroNet
LLC, Kristi is a registered nurse who limits her practice to brain injury care management,
treatment planning and residential placement.
- WAYNE EKLUND, RN:
Wayne provides superb life care plans for both children and adults with all disabilities,
as well as care management, expert witness testimony and other services related to his
registered nursing degree.
- PRAIRIE CREEK CASE MANAGEMENT LLC: Vivian Tillman has spent nearly thirty years
managing injured workers, catastrophic injuries, complex orthopedic injuries and related
conditions. Her coverage area includes central and eastern Oregon, Eastern
Washington and Idaho.
POST ACUTE REHABILITATION
- BRAIN INJURY REHABILITATION CENTER: is Portland's only freestanding post acute program
designed to care for brain injury survivors who are medically stable and preparing to return
to work or other vocational activities. The medical director is Danielle Erb, M.D.,
a physician who limits her practice to the management of brain injury rehabilitation.
DAY SERVICES
- COGNITIVE ENHANCEMENT CENTER provides day services for persons surviving acquired or
traumatic brain injury. CEC will meet or exceed the standards set by the National Adult
Day Services Association.
RESIDENTIAL CARE
- EXPRESSIONS OF LOVE: EOL is a five-bed Adult Foster Care program
in Oregon City that accepts both male and female brain injury survivors.
This program focuses on community reintegration and independent living skills in a
home like setting.
- HERON HOUSE: A five bed adult foster care located in Cornelius, designed for
brain injury survivors with complex needs such as behavior and mobility impairments.
- HIGHLAND HEIGHTS RESIDENTIAL CARE: Provides Level III adult foster care in Gresham
for medically complex survivors of neurological impairment, including seizure disorders,
coma management, end of life care, feeding tubes, etc.
- HILL HOUSE: A twenty-eight bed residential care facility housing survivors of brain
injury and related neurological conditions.
- HOMEWARD BOUND INC.: Homeward Bound is a five-bed Adult Foster Care residential program
in the rural community of Hillsboro, providing rehabilitation and life skills training for male
survivors of brain injury with mild to moderate behavioral dyscontrol. Homeward Bound
has been in existence for over fifteen years and has a proven track record of accomplishment for
successful clinical outcomes.
- LAKE OSWEGO RESIDENTIAL CARE: This is a nine-bed facility located in Lake Oswego, serving
individuals with neurological impairment over the age of fifty.
- RIDGEVIEW ASSISTED LIVING FACILITY: situated in beautiful southern Oregon is a 52-bed
facility designed for young, active residents, but is also able to accommodate those in wheelchairs.
Their registered nurse is available 24 hrs per day to assist with their medical and medication
needs.
COUNSELING AND BEHAVIORAL INTERVENTION
- ALLAN WOOD, LCSW specializes in family counseling for persons with severe disabilities
and behavioral disorders. His assistance is utilized by our residential care facilities
for training in crisis intervention, and behavioral management. He is also able to offer
grief counseling for survivors as well as their family members as they adjust to the disability.
VOCATIONAL REHABILITATION
- THOMAS WEIFORD, CRC, CCM, CLCP offers a wide range of services to our network of
affiliates. A certified case manager and life care planner, he also does vocational
testing and rehabilitation, as well as economic loss analysis.
FINANCIAL SERVICES
- BYRD FINANCIAL GROUP: Byrd Financial Group provides financial planning
services and special needs planning for families with survivors of catastrophic illness
and injury.
SEMINAR AND LECTURES
- We offer educational presentations on brain injury case management, anatomy, physiology,
as well as instruction and training for the American Academy for Certified Brain Injury Specialists.
PET THERAPY
- THE GOLDEN RETRIEVERS OF COPPER CREEK serve as pet therapy dogs for the various
facilities affiliated with NeuroNet. These friendly, outgoing dogs interact with
individuals who may have physical, emotional, and behavioral impairments to improve attention
and cencentration, encouraging speech and motivating simple physical activities such as petting
and brushing.
